Three Rivers Community Health Group complies with Title VI of the Civil Rights Act of 1964 (42 U.S.C. 200d et seq.), which states that “no person in the United States shall, on the ground of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance.” Any person who believes that discrimination has occurred by a state agency on the basis of race, color, or national origin, including limited English proficiency (LEP), in violation of Title VI may file a written complaint to the Title VI Coordinator, 115 East Brooklyn Street, Linden, TN 37096 or call 931-589-2104. Complaints must be filed within 180 days of the alleged discriminatory act.

This health center is a Health Center Program grantee under 42 U.S.C 254b, and a deemed Public Health Service employee under 42 U.S.C 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals.
